HOME AND COMMUNITY CARE
We are funded by the Home and Community Care (HACC) program and offer a package of 'home care' as an alternative to residential care. We provide assistance with tasks such as shopping, preparing meals, nursing, transport, home duties and personal care.
How can this programme assist you?
A member of the Coordination team will contact you and discuss with you and, if relevant, the person who cares for you, the support you need. We will:
- Negotiate a suitable package of care to meet your particular needs that is acceptable to you
- Keep in touch with you on a regular basis in case things change
- Advocate on your behalf as requested
- Provide information about available services and how to access them
- Discuss fees associated with the service
Who can contact this service?
Anyone can seek assistance, including:
- Individuals
- Carers
- Relatives and friends
- Doctors
- Other community service groups
Assessment prior to accessing this service is not necessary as your needs will be discussed with a member of the Coordination Team.
Who can be assisted?
The service aims to meet the needs of persons frail aged and younger persons with a disability for whom everyday tasks are difficult to manage and living in the following suburbs:
- Jolimont
- Wembley
- Floreat
- City Beach
- Daglish
- Shenton Park
- Mt Hawthorn
- West Leederville
- West Perth
- Areas of North Perth
In circumstances where a person cannot be accepted onto the programme, the Coordination team will provide information about other agencies that may be able to assist.
