Community Care Packages
A Commonwealth Funded programme aimed at providing a variety of services to enable persons frail aged and/or disabled to stay in their own home and maintain independence, as an alternative to going into low level residential care.
About the Programme
Services are flexible to meet the needs of the client and are monitored regularly by the Coordinator to respond to changing needs.
The client is a partner in care decisions and is treated with dignity and respect at all times. The client has a right to complain about the service and have an advocate on their behalf if desired.
A variety of services can be provided, including:
- Personal care
- Housekeeping/laundry
- Meal preparation
- Shopping
- Transport
- Socialisation
- Gardening
Fee
The fee for this service is currenlty set by the Governement at 17.5% of the pension, however your particular financial circumstances are taken into account when setting the fee.
How to access the service
Referrals are made by the nominated Aged Care Assessment Team. Any enquiries can be made directly to the Coordination Team.
