Before Your Admission
The Patient Pre Admission and Consent forms are available directly from your specialist, the Hospital or can be completed on-line.
When completing the form please include any relevant Health Insurance details and return these forms to the Admissions Department by post or fax at least four days prior to your proposed admission date.

Returning this documentation to the hospital prior to your admission will enable our staff to plan for your surgery, and confirm all pertinent health details.

Your specialist has booked your surgery and stay. It is not necessary to telephone the Hospital to confirm this booking, however if you have any questions please call us on (08) 9370 9222.
A Guide for Patients information booklet is available from our Admissions Department, if you would like a copy of the booklet please do not hesitate to call 9370 9222 and request a copy to be sent out to you prior to your admission. On admission patients are requested to bring health fund details and/or health subsidy cards. Please do not bring any valuables or jewellery with you.
The admitting nursing staff should also be informed of your health / medical history and any current medications should be shown.
Pastoral Care Staff and Chaplains are an integral part of the Mercy Health Care Team. These services are private and confidential, patients may elect to receive pastoral or spiritual support during hospitalisation.
The Pre-admission Clinic
The Pre-admission Clinic is a service provided for specific types of surgery and provides specific education and clinical preparation prior to admission into hospital. You will be issued with an invitation from your Consultant's rooms if this service applies to you.
What you should bring:
- Any letters, reports or medical records from your Doctor
- Any X-rays, CT or MRI films and reports
- Any medications you are currently taking
If you are staying overnight or longer, you should bring:
- Bed attire
- Dressing gown
- Comfortable leisure wear
- Personal toiletries
- Suitable footwear
We recommend that you bring suitable footwear to wear during your stay in hospital. Mercy Hospital Mount Lawley cautions patients against walking barefoot in the hospital or its grounds. Mercy Hospital Mount Lawley accepts no responsibility for patients choosing to disregard this caution and incurring any injury as a result of walking barefoot in any part of the hospital or its grounds.
If you are a day-patient, please wear comfortable clothing. Arrangements should be made with a friend or family members to drive you home after surgery.
Motel Accommodation
At nominal cost, semi-self contained motel units are available within the hospital grounds for friends and family, and are only minutes from the wards.

Rooming In / Children / Breast fed children / Boarders
Arrangements can be made for rooming-in for a parent or caregiver. Breast-feeding mothers who bring their babies to hospital are required to have an alternative carer available for the baby.
The hospital also has comfortable on-site motel units for relatives and friends of patients.
