What is the Home and Community Care (HACC) Program?
The Home and Community Care (HACC) program provides basic support services to some older people, people with a disability and their carers to assist them to continue living independently at home. This includes:
- support to participate in social activity in a group or one-on-one
- assistance with everyday household tasks
- assistance to enhance nutrition, function, strength, independence and safety
- assistance to support your independence in your personal care activities such as showering and dressing
- Assistance to keep up with essential activities such as shopping, banking and maintaining social contacts
Who is eligible for support from HACC?
HACC is designed to assist people with the greatest need and aims to maximise people's independence. You may be eligible for HACC if you:
- are older and frail and having difficulty with everyday tasks
- have a disability
- are the carer of a frail older person or someone with a disability
Who do I contact?
For further information contact a Commonwealth Respite and Carelink Centre on 1800 200 422 who will assist you with information about eligibility for community care services. You do not need a referral.
The Commonwealth Respite and Carelink Centres will establish your eligibility for support by the HACC Program.
HACC Regional Assessment Services in your area will confirm your eligibility for HACC support through a face to face assessment in your home that identifies your needs and goals or for more comprehensive support through another community service provider, or to MercyCare if you wish to have services provided by us.
What is the cost of a HACC service?
The HACC Program seeks a contribution from clients toward the cost of the support services provided that is fair and affordable. The contribution is based on the level of support that you receive.
A person receiving an age pension contributes a maximum fee of $58 per week. This is called the HACC fee cap.
No person will be denied a service because they cannot afford to pay.
Wellness Approach
Community Aged Care adopts a Wellness Approach to care. This means that our focus is on identifying what an individual 'can do' for themselves rather than focusing on what they 'can not do'. All our staff are trained in the Wellness Approach and promote and encourage client independence to enable our clients to stay at home longer.
The Western Australian Carers Charter
We recognise The Western Australian Carers Charter which is part of The Carers Recognition Act 2004.
It provides clear direction on how carers are to be treated and how carers are to be involved in the planning and delivery of services.
The Western Australian Carers Charter
- Carers must be treated with respect and dignity
- The role of carers must be recognised by including carers in the assessment, planning, delivery and review of services that impact on them and the role of carers
- The views and needs of carers must be taken into account along with the views, needs and best interests of people receiving care when decisions are made that impact on carers and the role of carers
- Complaints made by carers in relation to services that impact on them and the role of carers must be given due attention and consideration
