What you need to know before moving to an aged care home

Moving into a Residential Aged Care Home is a big life step. We want to make this transition as smooth as possible for you and your family.

Arrange an assessment

Before you can apply to enter a Residential Aged Care Home, you’ll need to be assessed as eligible by an Aged Care Assessment Team (ACAT).

You can arrange this directly with My Aged Care or you can get a referral from your GP. ACAT will visit you at home to ask you questions about your health needs and current lifestyle. They welcome you to have a family member or friend with you during the visit.

If you are assessed as eligible for residential care you will be provided with an ACAT Support Plan and can now apply to move into a MercyCare home.

Moving into your home

Our team will talk you through what you need to do to get ready to move in, while staff at your new home will be getting ready for your arrival to warmly welcome you. They will be there to make sure you are supported to settle into your new home over time.

We encourage residents to personalise their room. After all, it is your home. We provide an electric bed, bedside table and chair, but understand it is important to be surrounded by your own life and memories. We love to see you bring your own special possessions and decorations to personalise your space.
