Providing SUPPORT and OPPORTUNITIES for young people and their families.

 

Reconnect is...

an outreach support service providing assistance to young people (12 - 18) who are having difficulties staying at home, or who have recently become homeless because of family issues.

We work with young people, their parents /guardians and families to provide support through individual and /or family counselling, mediation, information, education, advocacy and referral.

Reconnect is a FREE and CONFIDENTIAL service. Reconnect is also a Voluntary service.

MercyCare Reconnect delivers services in the following local government areas:

North of the river

Reconnect is funded by the Department of Social Services (DSS).

Making a Referral

If you, or someone you know, is having difficulties staying at home, or have recently become homeless because of family issues, then assistance is available. Please call our Referral Officer on Freecall 1800 800 046. Using the Freecall number 1800 800 046 it is possible for young people and parents or guardians to refer themselves to Reconnect. Individuals or agencies can also make a referral by calling the 1800 number.

When a referral is made, clients are allocated a Case Worker who will usually make contact within the first two working days after contact. Service flexibility allows us to meet individual needs.

The referral line is open Monday to Friday 9am - 4pm and a response will be provided within 1-2 business days.
