Home Care Packages

Do you currently receive home care support, but think you may need more? Home support needs can change with circumstances. Home Care Packages are available to people who need increased support to live at home.

They can provide an alternative for people whose care needs are complex or require specific types of support which could otherwise see them entering a residential aged care facility.

The support offered as part of a Home Care Package will depend on individual needs, with choice and flexibility in the way support is delivered. The aim of MercyCare’s Home Care Packages is to assist people to remain living independently in the home and to give them and their families choice, flexibility and control in how home support is provided.

MercyCare delivers all Home Care Packages using a consumer directed care approach. Consumer directed care is a partnership model, where those providing support and those receiving support work together. People receiving support can choose the level of control they would like both in what support they receive and how they receive it.

A MercyCare advisor will support individuals receiving Home Care Packages and in partnership will discuss and outline a personalised approach to providing support.

How do you know if someone is eligible for a Home Care Package?

Home Care Packages are designed to be tailored to support people to manage a range of everyday tasks, in a flexible way. A Home Care Package can help you to stay at home and give you choice and flexibility in the way that support is provided to you. A range of services can be included as part of a Home care Package, including personal support services, social and community based supports and clinical care.

The first step to access MercyCare’s Home Care Packages is to be assessed by an Aged Care Assessment Team (often shortened to ‘ACAT’).

1. A referral is recommended for this assessment, so it is worthwhile discussing with your GP or other health professional.

2. Once a referral is made, an ACAT assessor will make an appointment to meet you in person.

3. The ACAT assessor will be able to offer guidance for appropriate support to suit your specific needs.

This is when you have the choice to request MercyCare to deliver your Home Care Package.

Is there a cost for a Home Care Package?

There is no cost for the ACAT assessment. As a contribution towards the cost, you will be asked to pay a basic fee of up to 17.5% of the single basic Age Pension. An income-tested care fee may also apply if your income is over a certain amount.
