Reconnect is an outreach support service which assists young people aged 12-18 years old who are having difficulties staying at home, or who have recently become homeless because of family conflict. We can provide counselling, group work, mediation, and practical support to the whole family.
Both the young person and a parent/caregiver must be aware of and consent to the referral.
For youth and family support, the Reconnect program uses early intervention strategies to address conflict, strengthen relationships and help to stabilise living circumstances, so that young people can focus on their education, training, employment, and future goals.
MercyCare offers this free service in City of Joondalup, Wanneroo, Stirling, Perth, Swan, Armadale, Gosnells, Cockburn, Fremantle, Kwinana, Rockingham, and Town of East Fremantle. Our qualified outreach workers can meet at a place and time which suits you and your family. If you live in the other Local Government areas, follow this link to get more localised support.
The Reconnect program is federally funded by the Department of Social Service.
The Mirrabooka, Joondalup Family support Network provides brief interventions for vulnerable children and families, young people aged up to 25 years and families involved with or known to multiple agencies.
This service involves assessing family needs and either coordinating a range of services to work with the family or linking the family with a partner agency who will coordinate the services they need. The Mirrabooka Joondalup Family Support Network works within a partnership approach working together with vital services ensuring each family or individual can select a mix of services whilst maintaining confidentiality.
