Depending on the level of support you need, My Aged Care will allocate you either the low-level support of the Commonwealth Home Support Program, or the higher-level Support at Home Package.

Many people use CHSP when they need low-level support. As their needs become more intensive or complex, requiring some case management assistance, they can be referred for a Support at Home Package. With CHSP your funding is only for specific services, like cleaning or personal care, and can’t be transferred to a different type of service without a reassessment. With a Support at Home Package, your funding is flexible and can be used on whichever services you require.

Generally, you must be 65 years or older (or 50+ for Aboriginal or Torres Strait Islander people) and need support to live independently at home.

You will need an assessment organised through My Aged Care. An assessor will determine your eligibility and the level of support you require.

Support at Home packages can help with a range of services to support independent living, such as:

Personal care
Help around the home
Grocery shopping and meal preparation
Transport to appointments and social outings
Nursing support
Allied health services
Carer respite

You can contact My Aged Care on myagedcare.gov.au or 08 9442 3498 . They will test your eligibility and arrange for an assessor to meet with you to determine your level of care.

If you have entry-level care needs, you’ll meet face to face with an assessor that will determine what services you require under funding from a Commonwealth Home Support Program.

If you have more complex care needs, you’ll meet with your local assessor who will determine what Support at Home Package level is right for you.

The Basic Daily Fee for a Support at Home Package is optional and can vary depending on your services and financial situation.

Paying this fee may allow you to access more hours of support through your package.

Once you have been assessed and approved for a Support at Home Package you can start looking for an approved home care provider.

When searching for a home care provider, important factors are choice, flexibility, compassion, professional staff and a proven track record. It is important to find a provider that suits you and your specific needs.

Our friendly team is here to listen to what you need and support you through the process, from beginning to end. Together, we will go over your needs, discuss services available, personalise services and develop a schedule that will support you to continue living independently at home and connected with your community. You will have full control to select services and design a schedule that meets your individual needs.

Once you give us the go-ahead, your services can start. We’re flexible, so if you need to change a service or time, just give us a call and we’re happy to help.

CHSP services are subsidised by the Australian Government and depending on your financial circumstances, providers may ask you to make a small contribution towards your services. Our friendly team are happy to chat through fees involved. Give us a call on 9442 3498.

Since things have been put in place Mum has been able to engage with others a lot more. Before she was missing activities because she couldn’t remember them.
