Start your home care application process
Unsure of your eligibility, or how to start your home care journey – whether that’s a Support at Home Package, the Restorative Care Pathway, or another service? We can help you understand your options and navigate the process every step of the way.
Below are some steps you can follow to guide you through the process. Our friendly team are only a phone call away to answer your questions and support you anytime throughout the process.
Arrange a home care assessment
Contact My Aged Care online or on 1800 200 422. They will test your eligibility and arrange for an assessor to meet with you to determine your level of care.
If you have entry-level care needs, you’ll meet face to face with an assessor that will determine what services you require under funding from a Commonwealth Home Support Program.
If you have more complex care needs, you’ll meet with your local assessor who will determine what Support at Home Package level is right for you.
If you need short-term, intensive support to regain strength or independence — for example, after an illness, injury, or hospital stay — your assessor may also determine you’re eligible for the Restorative Care Pathway. Find out more about Restorative Care.

Plan your home care support
Our friendly staff can sit down with you to go over your needs, discuss services available, and personalise your support – whether that means developing an ongoing care schedule, or setting clear goals as part of a Restorative Care goal plan.
Start!
Once you give us the go-ahead, your services can start. We’re flexible, so if you need to change a service or time, just give us a call and we’re happy to help.
Remember…
With a MercyCare Support at Home Package, we have no joining or upgrade fees – giving you the flexibility to change your services without being charged. (Note: this applies to Support at Home Packages. If you’re accessing the Restorative Care Pathway, your Restorative Care Partner can explain how your funding works.)
