A Home Care Package is a combined program of personalised care designed to help those with more complex needs stay in their home as they get older. Funded by the Australian Government, Home Care Packages are available in four levels with different hours of care and levels of support. With each recipient being allocated a budget to use on support and services, we can help ensure that you choose what is right for you and that these services are delivered effectively.
All of our home care packages put you at the heart of the process. We understand that everyone has different circumstances, desires and lifestyles; meaning that they need a care plan tailored to their exact requirements and goals. This could involve a multidisciplinary team consisting of a Case Manager, GP, nurse, occupational therapist and physiotherapist or exercise physiologist. These health care professionals can be engaged to make sure that every aspect of your care is assessed, planned for and properly delivered. It is all part of our mission to keep you happy and healthy in your own home for as long as possible.
Package value and costs
Each level of Home Care Package provides a different subsidy amount depending on your required level of care. If you choose MercyCare as your provider, we then deliver the services required to keep you living independently in your own home.
The four package levels are structured as follows:
- Level 1 – for people with basic care needs (e.g. help with house cleaning, meals, running errands and transport)
- Level 2 – for people with low-level care needs (e.g. help with daily living and professional medical support)
- Level 3 – for people with intermediate care needs (e.g. help with daily living and higher medical support)
- Level 4 – for people with high-level care need (e.g. access to high level and complex nursing care)
Who is eligible?
Depending on your age and individual circumstances, you may be eligible for government funding support to help cover the cost of your home care needs. This funding is provided with the aim of allowing as many people as possible to continue to live rich, fulfilling lives in the comfort of their own homes for as long as possible.

How do I access a Home Care Package?
As part of the home care package process you are able to choose who you want to provide your package. You can request MercyCare as your provider during your assessment with the Aged Care Assessment Team and we can then come to visit you to put the services in place. We can also attend your meetings and assessments with you to make the whole process as easy and simple as possible.
